Employer application form

Apply here on behalf of your employees for the Basic, Standard or Enhanced Certificates.
Note: Only Basic Checks can be provided for jobs located outside the UK if the employer is not a UK based company.

Overview of the application process

1. All details of the Employer application form are completed and the declaration must be read and understood by the nominated person.
2. Name and e-mail address and position title of all individuals to be checked will be provided and the level of Disclosure for each individual selected. The “Certificate Type Finder” can be used to determine the right level of Disclosure within less than 1 minute.
3. Upon selection of the preferred delivery method, you will be directed to our secure payment system.
4. After the payment has been submitted, the hard copy application forms will be posted to the address provided and applications eligible for online processing will be processed.
5. Hard copy application forms received by the employer will be handed to the individuals for completion and personal documents will be checked by the designated evidence checker.
6. In case clarifications are needed during the process of completing the application forms, we will gladly provide our assistance.
7. Upon completion of the application forms, they will be sent to Global Choices for further processing.
8. Once the Disclosure Certificates are issued, one copy will be sent to the individual and one copy will be sent to Global Choices and forwarded to the employer.

The standard processing times are as follows:

• Enhanced level: 4 weeks
• Standard level: 2 weeks
